To encounter Xesht, you must enter the Twisted Domain, accessible via the Realmgate on the Atlas by using a Breachstone. The Realmgate is conveniently located near the starting point on the Atlas, easily found by clicking the home icon to focus the camera.

How to Get a Breachstone in PoE 2
There are two primary methods to obtain a 
Breachstone in PoE 2:

Can You Use Orb of Chance to Get Hand of Wisdom and Action
No, you cannot use an 
Orb of Chance to obtain Hand of Wisdom and Action. HoWA is a limited drop from Xesht, We That Are One, and does not appear on the regular loot table like Astramentis, 
Headhunter, or 
Polcirkeln. Therefore, attempting to use an Orb of Chance on White Furtive Wraps will not yield HoWA.
